35세 남성 환자로 내원 3일 전부터 소량의 연변과 제부 주위 둔통이 발생하여 인근 의원을 방문하여 장염 진단 하에 약물치료를 하였으며, 내원일 오전부터 하복부 통증으로 발전하였다.

내원 시 vital signs은 정상이고 하복부 압통이 있으나 국소적 압통보다는 범위가 넓고 우하복부에서 방광 주위까지 퍼져 있었다.

▶ laparoscopic appendectomy 시행
1. 수술 소견
1) severe necrotic change on appendiceal tip
2) intact appendiceal base
3) micro perforation on appendiceal tip
4) 7.5×2.0 cm size

2. 수술 후 진단: Perforated appendicitis with periappendiceal abscess

▶ Surgical pathology report
1. Gross: The specimen received in formalin is a product of appendectomy measuring 7.0 cm in length and 0.6 cm in diameter. The wall of appendix appear to be edematous and focally covered with suppurative exudates.

2. Micro: The section shows acute appendicitis. Many acute inflammatory cells are found in the muscular coat of appendix. The appediceal abscess is also present.

3. Pathological diagnosis Appendix, appendectomy:
(1) Acute appendicitis,
(2) Periappendiceal abscess
